One type of form contains information on the Society's annual accounts and these have been photocopied and listed as DD/796/4403-5. Another significant type of form contains not only details on fees paid to the society but also details including the individual's health, occupation, spouse, etc. Many forms of the men's society are also accompanied by medical certificates |
